Massage Therapy for All Bodies
These are forms that I need for treatment. Everyone fills out an Intake Form. The Physician's Permission Form is only necessary in specific situations; I'll let you know beforehand if I need one from you. You can print these out in advance, fill them out, and bring them to the session with you. Especially in COVID-19 times, this reduces potential transmission as well as bringing a general ease to getting started.
This form gives me basic health information and also contains written consent for treatment with me.
Physician's Permission
If you have a condition that requires a doctor's release before I can safely administer massage, you will use this form. You can print it in advance and have your physician fill it out. I will need a legal physician's signature on the form before I can continue treatment.
